You have completed your cancer treatment and likely have a new outlook on life.

We know there is a sense of comfort meeting regularly with your healthcare team at Illinois CancerCare, P.C.

We also realize being a post-treatment cancer survivor brings a new set of questions and uncertainty as you continue your cancer journey. It’s our goal to make life after cancer the best it can be.

Our Cancer SurvivorCare program is about more than surviving cancer. It’s about individualized care that will follow you the rest of your life by connecting you with a team of clinical experts specializing in post cancer treatment and qualified to address concerns you may have.

Personalized Care

With this program, you will receive the same personalized, comprehensive, high-quality care you received during your cancer treatment. As you transition from active treatment to the next steps in your cancer journey, it’s about living a new normal with an informed, all-inclusive approach to your health and quality of life.

We created the Cancer SurvivorCare program because we know for survivors and their loved ones, the cancer journey extends beyond cancer treatment. At Illinois CancerCare, we are always looking for new ways to improve your physical and emotional health.

Our approach to Cancer SurvivorCare is based on a true partnership between the survivor and their healthcare team. Once your cancer treatment is complete, you will continue to see your oncologist for post treatment follow up appointments. When your oncologist feels you are ready and you are comfortable, you will meet with our Harvard-Trained Cancer Survivorship Specialists. This physician has a great relationship with your oncologist. Our team will work together to ensure you continue to receive the car you need exactly when you need it.

The first step is an individual meeting to assess medical, personal, emotional, social and economic concerns as well as strategies for maintaining long-term health and well-being. Your CancerCare Specialist will work together with your oncologist to ensure you receive the care you need exactly when you need it.

Beginning your Post Cancer Treatment Journey

The first step of our Cancer SurvivorCare program is an individual meeting with a Survivor Specialist to assess medical, personal, emotional, social and economic concerns. You will also work together to strategize a plan to maintain long-term health and well-being.

Regular physical exams with an overview of your recent medical history
Management of cancer and treatment-related side effects
Referrals to community resources and support groups
Intimacy and sexuality concerns
Risk for developing another cancer
Quality of life issues
Pain management
Monitoring for early detection of new or returning cancers
Lifestyle interventions to reduce cancer risks
Fertility issues
Genetic risk of cancer among family
Pre and post Rehabilitation
Nutrition services to help develop and/or strengthen healthy eating habits.
