1) Remember to bring your completed information packet for your first visit.

2) Bring a current list of medications and include the dose and frequency of consumption. Peoria clinic patients may meet with our pharmacist to review your prescription history.

3) If you have diagnostic tests or x-rays at other facilities, be sure to notify Illinois CancerCare's new patient scheduler so that these records can be obtained.

4) Also remember to bring your insurance cards. You will meet with our financial counselor to help answer any of your insurance or financial questions.

5) You are encouraged to bring a family member or friend with you.
